Athletics Eligibility

Please visit the following page to register for all sports, including Cheerleading, for 2026-2027 beginning on May 1, 2026. Please click on all sports your student is interested in participating for the 2026-2027 school year.

• Semester 2 grades from 2025/26 from Middle/High School for ALL fall sports
2.00 unweighted GPA MINIMUM
           ▪ no more than 1 F
           ▪ no more than 1 UN citizenship grade

• Register for athletics starting in May 1, 2026 for EAHS. Use your School Email log-in for
registration.

• Get a Sports Physical ASAP!
Physical form is online and onsite physical offered 1st week of school for ALL SPORTS!
Physicals are good for 1-calendar year from the exam date. Download form HERE.

• If you are an incoming Freshmen or new to athletics in high school, you’ll need to
complete the drug contract presentation BEFORE you can get cleared. Sign-up and
complete via link: https://edpuzzle.com/join/dokupmu Use your School Email log-in
for registration.

• If you are in incoming Freshmen or new to athletics in high school, you need to
complete the ImPACT test. If you took this and it’s been a couple years, you’ll also
need to do this test again. Please follow these directions:
   1. Go to www.impacttestonline.com/testing
   2. Enter the code BDWRR4Z8BC and select your organization for
          ▪ “Everett Alvarez High School”
   3. Go to Launch Baseline Test
          ● Select desired language
          ● Hit OK
          ● Hit NEXT
          ● Fill in all personal information and then Hit NEXT
   4. Take the test. DO NOT RUSH IT and do it in one sitting undistracted or it will
come back invalid and you’ll have to do it again.
   5. Email AD Vazzy that you have taken it with a screenshot of your completion
confirmation you get at the end.
